Who is the H2H program for?

The H2H program is specifically designed for NDIS participants, including children (7-17 years) and adults, who are being discharged from the hospital with ongoing support needs. We provide care for NDIS participants with conditions such as:

  • Complex Mental Illnesses (E.g., Schizophrenia, Bipolar Disorder, and Severe Depression)
  • Acquired Brain Injury (ABI)

How the Hospital to Home program works

Our H2H program is designed to ensure continuity of care and a successful transition to your home. We provide comprehensive care coordination, where a dedicated program coordinator will liaise between the hospital, patient, family, and community providers, and work closely with your hospital care team to create a personalised discharge and care plan. Lifest HealthCare staff also provide ongoing personalised care in the home and community support, and offer assistance with medications and medical and allied health therapies. We help with meal preparation, daily living tasks, and participation in community activities, as well as capacity building to promote independence and long-term wellbeing.

Our specialised hospital to home transition process is carefully structured into three key stages:

 

Stage 1: Discharge planning

  • Set a discharge date with your hospital team.
  • Obtain medical reports to develop a care and implementation plan.
  • Identify necessary support providers and accommodation.

 

Stage 2: Plan implementation

  • Develop a detailed transition plan.
  • Introduce support staff to build rapport before discharge.
  • Provide specialised training for staff on medical and behavioural care needs (e.g., medications, PEG feeding, ventilator care, catheter management, bowel care, wound care, dialysis).
  • Conduct short visits to the new home and community to familiarise you with your surroundings, connect with a GP and pharmacy, and arrange trial overnight stays.

Stage 3: Discharge and ongoing care

  • A registered nurse will take over from the hospital discharge nurse.
  • Our support staff will transport you home and help you settle in.
  • The program coordinator oversees the transition for 1-3 months or more, ensuring all stakeholders are informed and that your care plan is successfully implemented.
  • Continued support through our recovery maintenance program.
