Social And Community Access in Brisbane

NDIS Social And Community Access

At Lifest HealthCare, we believe living a fulfilling life is about getting out and about, meeting new people, trying new things, and experiencing the world.

Social and Community Access is a funded support in the NDIS package that focuses on goals that are related to participation in social groups, community events, and involvement in recreational activities. Our support team will assist you in gaining access to your local community to increase your sense of belonging, connection, and inclusion.

This service includes, but not limited to:

  • Travel Assistance
  • Recreational activities engagement
  • Attending allied health / GP appointments
  • Shopping assistance
  • Visiting family and friends
  • Attending neighborhood events
  • Playing sports

Individualised Living Options (ILO)

Independent Living Options are designed to create a sense of belonging and to build your independence.

Individualised Living Options are customisable support packages that can be interchanged to suit you and your unique needs. ILO services include support with daily living tasks, assistance in developing your skills, or assistance with household tasks like meal preparation and shopping. Under this option, you have the freedom to choose how you want to live, where you want to live, and the type of support you need on a daily basis. If ILO is not part of your funding, at Lifest HealthCare we have introduced a unique in-house package called Flexible Living Option (FLO). FLO is similar to ILO services, it allows you to access supports you require to live independently in the house you choose.

Supported Independent Living (SIL)

In Supported Independent Living, Lifest HealthCare assist our participants with tasks of daily living to support and promote independence.

We adapt to individualised, person-centered care that is tailor-made to support participants needs, goals, and choices. We aim is to help you live as independently as possible while building your daily living skills – such as cleaning, medication management and food preparation. We have SIL houses available across the Greater Brisbane Region – get in contact with our team to know more about our current vacancies.

Our friendly and dedicated staff are well trained in delivering 24/7 disability support for our participants with:

  • Daily Tasks: Assisting participants with personal care and mobility.
  • Life Skills: Facilitating to learn new life skills.
  • Behaviour Support: Providing individual support to live a happier & healthier life.
  • Housekeeping: Supporting participants with meals preparation, cleaning, and other essentials.
  • Activities: Exploring hobbies and interests with participants
  • Medication Assistance: Assisting participants with taking medications

Allied Health

Our dedicated Allied Health team provide quality care to participants to achieve their full potential. Our team is composed of – Physiotherapists, Counsellors, Occupational Therapists, Psychologists, and Registered Nurses.

Nursing Care:

Our experienced nursing team is dedicated to providing quality care to participants with disabilities. We provide the following: 

  • Incontinence Assessments
  • Non-Invasive Ventilation Support
  • Invasive Ventilation Support
  • Catheter Care
  • Vital Sign Observation and Management
  • Percutaneous Endoscopic Gastronomy (PEG) cares
  • Diabetes Management
  • Wound Management
  • Delegated Health Care Plans
  • Training of support workers
  • Medication management

 

Physiotherapy:

Our Physiotherapists help our participants manage chronic conditions, give lifestyle advice and prescribe exercises to improve and support daily living. 

When our physiotherapists come to visit you, they might: 

  • Therapeutic massages 
  •  Manipulate Joints
  •  Stretch your muscles
  •  Provide and recommend exercises for you to do

 

Occupational Therapy (OT):

An Occupational Therapist focuses on promoting a participant’s health and wellbeing by enabling them to participate in everyday occupations of life. Working alongside both physical and mental health illnesses and disabilities, some key areas of activities where occupational therapists may provide support include: 

  • Self-care Activities: showering, dressing, food preparation, house tasks 
  • Productive Activities: Attending school, work, volunteering, budgeting  
  • Leisure/Social Activities: Being a member of a community group, engaging in a hobby, socialising with friends

 

Counselling:

Our counsellors have the capacity to work with you one-on-one or in group sessions to work towards developing healthier relationships and improve your overall wellbeing. Whether you are experiencing feelings of anxiety, depression, angriness, confusion, or generally overwhelmed, our counsellors can help you implement coping strategies to improve your confidence. 

 

Psychology:

Our psychologists provide services spanning one’s life. We provide evidence-based intervention for a variety of common difficulties experienced in our participants’ lives. These include: 

  •  Anxiety
  •  Depression
  •  Stress
  •  Adjustment to new situations

We focus on improving social skills, mood management, and emotional regulation to help our participants heal and become aware of their inner strengths.

Specialised Disability Accommodation (SDA)

Our NDIS Specialised Disability Accommodations, include a range of housing options that are designed specifically for our participants with extreme functional impairments or high support needs. The aim of our SDA is to make accessing support easier.

SDA is a housing option designed for people with high support needs or functional impairments. All our SDA homes are tailored to meet the specific needs of our participants.

Our SDA is built with disability access, assistive technology, hoists, and specialised home modifications – offering a safe and comfortable space to live while receiving personalised support from our experienced and caring staff.

Our SDA’s are designed and operate according to the NDIS design standards and operational guidelines – covering the four main NDIS requirements:

  • Improved liveability
  • Robust construction
  • Fully Accessible
  • High Physical Support

Community Centre

Our Lifest HealthCare team bring to you, our Shine House, located in Crestmead. At Shine House, we believe that life amounts to much more than just the serious stuff – it is sometimes as simple as coming together, finding a safe space, and growing as individuals.

Our activities team at the community centre hold activities and special events tailored for all to enjoy in an inclusive and engaging environment. Our community centre activities mainly emphasise on learning practical skills, social skills, and building confidence.

Throughout the week you will be able to learn new skills, meet new friends, and most importantly – have fun. At our community centre we have the following activities:

  • Art & Craft
  • Hydrotherapy (Co-ordinated by our Therapy Assistant)
  • Personal Training Sessions
  • Fitness Classes
  • Outdoor and Indoor games
  • Dancing workshops
  • Men’s Shed (Woodwork, Refurbishing Furniture, Mental Health Discussions)
  • Culinary teachings through cooking, meal preparation, nutrition and safe food handling.
